All patient appointments are required for in office visits

New Patients

If you are new to our practice, please plan to arrive 15 minutes before your scheduled appointment time in order to verify completion of online patient intake forms. This will allow us to establish your medical file before you see your doctor.

You will need to complete our patient intake forms in the Patient Portal. After scheduling your appointment and talking with the office staff, you will receive an email invite to register on our Patient Portal. If you did not receive this email or are unable to access the portal, please contact us so we can assist you.  Completion of the online patient intake information is required prior to seeing the provider.  Incomplete forms will require your appointment to be rescheduled.

If you are planning on filing for or are currently on disability, this office cannot accommodate disability requests.


** Patient Responsible payments are collected the prior day.   This allows for lowering the administration in the office and focusing this time to respond to patient needs.  Big Thank You to everyone for supporting the process!!  It is a big help for us.

** Cancellation Policy.  Please make sure you review.  We do understand that things come up.  Please let us know as soon as possible if a change is required.  There are many patients that want to get in to see the providers.  Short notice cancellations do not allow us to be able to provide an opportunity to the other patients.  We wanted to make sure you knew how important your scheduled appointment is and to please think of the others.
